Required Forms
To ensure we remain in full compliance as a North Carolina state-registered K-12 private school, the following documentation must be on file for every student.
Please submit the following items to the school office:

Official Records: A copy of the student’s birth certificate and up-to-date immunization records.

Contact Information: A completed Personal/Medical Phone contact form.

Consent & Agreements: Signed copies of the Photography Consent, Technology Agreement, and Cellphone Policy.

Important Deadlines

Initial Submission: All forms must be submitted no later than the first day of school.

Annual Renewals: Updated information and agreements must be submitted at the start of each academic year.

Mid-Year Changes: If your child’s medical or contact information changes during the year, please provide updated documentation immediately.
